This year, our grantees and partners achieved impressive results to improve the health of Michigan residents. We were thrilled to support this work while serving as a convener and connector to help partners learn, adapt, scale, and collaborate together. In our 2024 Year in Review, we reflect on stories and notable outcomes that we were proud to be a part of this year:

Technology has the potential to help rural Michiganders reach care providers more easily, which is why the Health Fund supported the Michigan Center for Rural Health in implementing a remote patient monitoring pilot aimed at increasing digital health capacity and access to care for rural, independent critical access hospitals. The pilot concluded earlier this year and provided learning opportunities for rural health stakeholders throughout the state. Read more:

Having support from a doula can lead to better birth outcomes for infants and their mothers. That’s why our Maternal & Infant Health Initiative has made increasing access to doula care a key priority, especially for families that would benefit most from this support. We’ve seen some major recent developments in Michigan aimed at expanding access to doula care — our blog shares examples of how our grant partners are leading and leveraging these efforts in their communities and statewide:

Nonprofit leaders bring an important voice to public policy discussions, which have a powerful influence on the health of Michigan residents. The Health Fund partners with Center for Health and Research Transformation (CHRT) to include staff from health-focused nonprofits in CHRT’s annual Health Policy Fellowship. This experience gives fellows a broad education that helps them understand and engage in the policy process. Our recent blog post shares how our nonprofit partners are using the fellowship to build collaborative campaigns and advocate for the communities they serve.

View the RFP and attend our "two-in-one” webinar on January 6, 2025, to learn more about program priorities, processes, and updated features. We'll start with a short session on preparing budgets for Health Fund grant applications. Visit our website for details:

On Thursday, we were thrilled to host an amazing group of Community Health Workers, organizations that support and deploy CHWs, and MDHHS representatives for a focus group at the Health Fund office in Lansing. CHWs are a powerful force for improving community health, increasing access to care, and addressing health related social needs. The focus group helped our staff learn more about challenges organizations face in sustaining CHW services, along with opportunities for the Health Fund to work with partners to address those challenges. Huge thanks to our guests for sharing their experiences and wisdom with our team! People with lived experience of food insecurity have unique and valuable expertise about solutions to support the nutritional needs of families like theirs. That’s what made the University of Michigan School of Public Health's Community Food Advocacy Fellowship (CFAF) so impactful and why the Health Fund supported its mission. The six-month fellowship brought together 19 Michigan parents to learn together and apply their experiences to influence food access policies, practices, and programs within their communities. People with at least one grandchild are more likely than those without grandchildren to say they hardly ever feel isolated. That’s just one finding in the latest poll extra from the National Poll on Healthy Aging, which looks at the experiences of grandparents and their families, including the connections between grandparenting and mental health. The poll also utilizes Health Fund support to spotlight state-level data about grandparents in Michigan. Check out the full findings:
