Casa de Esperanza is one of the first resources of its kind in Tarrant County. This partnership delivers healthcare directly to individuals who need permanent supportive housing. JPS has implemented Community Health Workers at Case de Esperanza, who help bridge the gap between healthcare and homelessness for residents.
