In the linked JAMA Neurology article, Drs. Grouse and Esper do important work to make the case that ballooning patient portal messages are substantially impacting provider workloads in #neurology and beyond. But in focusing on strategies to better manage an avalanche of messages, have they missed an opportunity to reduce that volume in the first place? Our work with providers around the country - including neurology practices large and small - has shown that effective, scalable Chronic Care Management programs can enable a large number of tasks to be handled by clinical staff before they ever hit the physician's radar. (We're also significantly reducing the call volume for nurse triage teams.) CCM can make clinical staff billable, generating substantial reimbursement (enough to hire more staff to help). But most importantly, engaging patients on a continuous basis (for us, that means 70-90% average monthly patient engagement with 3.4 average conversations/month) means that care managers can proactively identify health risks and intervene early. Once a patient reaches End Stage Kidney Disease, treatment options are fairly limited and terrible (dialysis or transplant). And most patients wait years for a transplant - 90,000 currently on the list. Of those, 13 will probably die today - even though 30% of donor kidneys will be discarded this year It's tragic, and terrible, and a major CMS focus.
