About us
Home Health Companions is a licensed home health provider offering in-home companion and caregiver services, private duty nursing and aging life care services. Specialized care for clients with Alzheimer’s, dementia, Parkinson’s and cancer. We serve clients in 13 D/FW-area counties. Compassionate caregivers and nurses deliver health and daily living services 24-hours a day, seven days a week. Care plans are custom tailored to meet a range of needs, like simple companionship and conversation, meal preparation, light housekeeping, shopping, transportation and medication reminders. Certified nursing assistants can help clients recover after hospitalization and avoid hospital re-admission. Our licensed nurses provide one-on-one medical care, responding to changing health needs while working closely with the client’s medical team to provide the appropriate level of care. Care can include: medication management, post-operative care, pain management, wound care, diabetic care, heart disease and hypertension, respiratory therapy, stroke recovery, incontinence care and Foley / colostomy care. A certified Aging Life Care professional, also known as a geriatric care manager, is on staff and available to guide families through all care options, selecting the appropriate choices for the best care possible, removing worry and stress. Monitoring health and observing mental and behavioral changes creates a safe environment that allows clients to age in place. Care Management • Hourly (days, evenings, overnights, weekends) around the clock or live-in care • Short-term care following a hospital stay • Temporary or long-term care giving • Medication Reminders • Transfers • Respite care for a primary caregiver or family member • Travel companion • Assistance with ADL's (Activities of Daily Living) such as personal hygiene and grooming, dressing, light housekeeping, cooking • Transportation • Companionship for the more active adult • Alzheimer's and Dementia care • Parkinsons
