We were honored to be invited to participate and donate to the Jackson Health System Prescribed Pediatric Extended Care Center Kids Holiday Event last week in Miami! 🎁 Jackson Pediatric Center provides quality skilled nursing care with intensive early intervention to children with special medical needs, and helps get them accustomed to a quality of life that will allow them to be integrated back into the community and the school system. Through community donations, vendor donations and Get Involved Now, Get Well's nonprofit, these kids received gifts aplenty and a special visit from Santa. Thank you for inviting us to celebrate with these resilient children and allowing us to witness the magic of the holidays through their eyes! ✨ Learn more about JHS here: https://lnkd.in/eR8HdKBR #PediatricCare #GetInvolvedNow

Preventable pediatric readmissions cost children’s hospitals and pediatric providers a staggering $1.5 billion annually. Find out how digital patient engagement can reduce readmissions by supporting families early on with education, reminders, follow-up, escalation to care when needed – and by addressing their SDOH needs. https://lnkd.in/eu7qJ3sb #PediatricCare #PatientEngagement #SDOH #HealthcareInnovation

🫶 We're beyond honored and proud to recognize Carlos Tafolla, a Get Well Navigator at Adventist Medical Center - Hanford for hitting an incredible milestone of activating his 4000th patient onto our app this week! 🎉 The Adventist Hanford team took time out of their day to celebrate and acknowledge Carlos for his impact. Carlos is such an integral part of the Hanford team and helps drive and achieve Adventist's Get Well goals for BYOD - we would be lost without him! 💜 Our navigators are deeply embedded in the communities they serve, and we are also proud to share that the same team pictured here is the team that helped take care of Carlos' dad as a patient during the week of Thanksgiving. Such a special, full circle moment. Thank you Carlos and team Adventist Hanford for your dedication to patient engagement!
