We offer Medicare Advantage, Senior Care Options (SCO), MassHealth Accountable Care Organization (ACO) Partnership Plans, Programs of All-Inclusive Care for the Elderly (PACE), and a limited number of products on the Massachusetts Health Connector, including ConnectorCare. Fallon Health has two Programs of All-Inclusive Care for the Elderly (PACE), alternatives to nursing homes that provide older adults and their caregivers innovative health care and support. In Massachusetts, Fallon Health offers Summit ElderCare. In New York, Fallon offers Fallon Health Weinberg–PACE in partnership with the Weinberg Campus. Since its inception in 1977, Fallon Health has worked to improve the quality of life and the health status of the diverse communities we serve. Our community engagement efforts are driven by the needs of the community and fueled by a deep commitment to the mission of Fallon Health. We support programs that promote equitable health care and social services. We also think community service is so important that we encourage our employees to volunteer. Fallon invites employees to volunteer one day per year, on company time.

Thank you to the Fitchburg Housing Authority for inviting us to the Wallace Tower groundbreaking ceremony. We’re proud to partner with you to integrate health and long-term care services in housing, helping more individuals, including our Summit ElderCare PACE participants, age in place. Congratulations on this forward-thinking initiative! https://lnkd.in/exM4WF9E

Interoperability is key to creating a more efficient, effective, and patient-centered health care system for all. And Fallon Health is proud to be at the table on this important topic. Thank you, John, for your leadership in this area of focus for our organization.
💡 In our new customer spotlight series, we’re shining a bright light on healthcare interoperability leaders. We’re starting with John Budaj, Vice President of IT at Fallon Health. We recently had a chance to sit down with John and ask him a few questions about his career path, approach to interoperability, and technology strategy, among other key topics. When asked how CMS interoperability mandates (CMS-9115, CMS-0057) influenced his strategy, he said, “Generally speaking, when it comes to mandates, you want to comply and meet the letter of the law. However, in this case, compliance alone may not be enough. We need to look at this as the jumping off point to bigger and better opportunities to engage with our provider communities that will have positive impacts on the communities we serve.” We hope you learn as much from John as we did. Research shows that Programs of All Inclusive Care for the Elderly, which integrate behavioral health services into primary care, significantly improve patient outcomes. With nearly 30 years’ experience offering this innovative model of care, our team was honored to lead a discussion on this topic at the 2024 National PACE® Association Annual Conference. Our PACE experts Dr. Jean Jaoude, MD, Annamaria Stamos, BSW, MPA, and Kevin Malone, JD, shared strategies to improve outcomes for older adults with psychiatric diagnoses, helping other PACE providers better serve this population. Thank you all for your leadership!

Our Summit ElderCare PACE Centers celebrated #NationalPACEMonth this September by asking participants, “How does PACE simplify your life?” Their comments were written on festive cutouts for the bulletin boards. Here's what they shared: • “I love the socialization at Summit.” • “I’m seen by staff right away.” • “It’s my happy place—great friends and supportive staff.” • “They make sure I have everything I need.” • “PACE is caring, loving, and patient.” We’re grateful for these insights and remain dedicated to making a difference in our participants’ lives.

As the first health plan in the country to offer a Medicare Advantage plan, we understand that health care is personal and your needs are unique to you. From now until Dec. 7, you can make changes to your Medicare coverage. Our team is here to help and can talk you through the process. https://lnkd.in/e6bc5RdG

Thank you Alliance of Community Health Plans (ACHP) for an excellent symposium on the future of health care. Fallon Health's president and CEO Manny Lopes spoke on the panel about health disparities and trust during the Health Equity Keynote Fireside Chat. He emphasized the voices of the community in the solution development process by powerfully stating “'Nothing for us without us. We know the problems we’re facing, and we should be part of that solution development.'" Special thanks to Ceci Connolly, president and CEO of ACHP, for moderating this meaningful discussion.
Fantastic industry insights and a diverse dialogue at this year's #ACHPSymposium event! Together, we can find practical solutions to our biggest health care challenges.

We’re pleased to share our MassHealth ACO plans are among the best in the country. In NCQA’s 2024 Health Plan Ratings: Medicaid, Fallon Health once again achieved 4.5 Stars, earning us a spot among the top 14 plans nationally. No other Massachusetts Medicaid plan is rated higher. The achievement speaks to the quality of the products and services we offer and the work we do to best serve our members. Ratings are based on the quality-of-care patients receive, how satisfied patients are with their care, and health plans’ efforts to keep improving. Learn more here: https://lnkd.in/eb2v2-Du

We proudly joined #PrideWorcester to support the LGBTQ+ community and encourage conversation about inclusivity in health care. We asked people to write down their thoughts to our question, “How can your health care providers make you feel more welcome or be more inclusive?” We’re committed to using this feedback to drive real change.
