While older drugs could cause issues like kidney stones, sedation, and mood changes, the newer ones are generally well-tolerated. Some people might experience minor constipation or injection site reactions, but overall, these medications are designed to minimize disruption to daily life. Although insurance coverage can be a hurdle, these newer CGRP drugs are becoming increasingly recognized as a first-line treatment for migraine prevention. Dr. Suneja from the Cleveland Clinic explains how overusing common pain relievers, like acetaminophen and ibuprofen, can actually worsen headaches over time. This happens because your brain can become dependent on these medications, leading to a condition called "medication overuse headache". As a general rule, limit over-the-counter pain reliever use for headaches to no more than two days a week and 10 days a month. Even prescription medications like triptans should be used mindfully. It's important to recognize the signs of medication overuse headaches and consider a "medication detox" to give your brain a break and allow preventive treatments to work more effectively. While most patients with typical migraines don't require an MRI, there are certain situations where it may be necessary. If your headaches change in pattern, become worse with exertion like coughing or sneezing, are positional, or if you experience vision abnormalities, it's crucial to discuss these symptoms with your healthcare provider. They may recommend an MRI to rule out any underlying causes. Although most MRIs in migraine patients are normal, occasionally small white spots or "scars" from migraines might be seen. However, these are usually benign. In some cases, the MRI can reveal other conditions like multiple sclerosis or chronic ischemic changes, highlighting the importance of a comprehensive evaluation along with the MRI findings. However, the most common and cost-effective method currently used is cognitive testing, such as the Montreal Cognitive Assessment (MOCA) and the mini-mental status exam, which can be administered in a doctor's office to screen for early signs of cognitive decline. EMTs can assess the patient on-site, rule out other conditions like low blood sugar, and begin treatment right away.
