Intraoperative hypotension is common and can lead to myocardial injury, AKI and mortality, which can impact hospital costs. One study found that IOH can increase hospital host between $1.2 - $4.6 million per year for a hospital with an annual volume of 10K noncardiac surgery patients. The study only included costs for AKI and myocardial injury and did not include readmission costs. Hypotension in the perioperative period is common. 44% of noncardiac surgery patients experience IOH with a MAP under 65 mmHg for over 10 minutes. And 48% of noncardiac surgery patients with IOH and 22% of patients without IOH experience postoperative hypotension of a MAP <65 mmHg within 48 hours of surgery. This increases the risk of perioperative morbidity and mortality including acute kidney injury (AKI) and myocardial injury. Many times, intraoperative BP is only checked every 2-5 minutes intraoperatively and every 15 minutes to 1 hour postoperatively using an oscillometric blood pressure cuff. This can result in undetected BP drops that increase the risk of poor outcomes for military veterans. Fluid administration patterns can vary widely in the absence of goal-directed therapy protocols which may contribute to variability in surgical outcomes.
