Up to 2/3 of tibial lesions treated with balloon angioplasty develop restenosis within the first 3 to 6 months . Contemporary studies with PTA as primary treatment reveal suboptimal procedural and 1 year outcomes. Restenosis leads to worse ambulatory status, more incomplete healing and worse resolution of rest pain. Specific devices for the BTK segment are warranted, including devices for preventing recoil, treating dissections, and preventing neointimal hyperplasia with proven DCB technology to optimize durability of endovascular interventions and ultimately improve the limb-related outcomes with CLTI patients. This case of a CLTI patient shows a BTK lesion preparation strategy with the Serranator balloon followed by Acotec Litos DCB angioplasty (3.0x200mm). !No vessel wall dissection or recoil answer after scoring balloon angioplasty! "Litos DCBs strikingly reduced LLL, vessel reocclusion, and clinically driven target lesion revascularization compared with POBA in BTK angioplasty.
