New FDA Draft Guidance on Tissue Biopsies in Clinical Trials The FDA has released a Draft Guidance to help guide industry professionals, investigators, institutions, and Institutional Review Boards (IRBs) on the important considerations when including tissue biopsies in clinical trials. Whether working with adult or child participants, biopsies can be critical in obtaining data for clinical trials, but they also introduce risks that must be carefully considered. Key highlights from the guidance include: Scientific Justification: Biopsies should be clearly justified in the clinical protocol, especially when needed to assess primary endpoints or determine eligibility. Risk Consideration: The risks of performing biopsies must be weighed against the anticipated benefits, and participants must fully understand these risks before consenting. Children's Safeguards: Additional ethical protections are emphasized for pediatric participants. If biopsies are conducted, they should offer direct benefit to the child or be limited to minimal risk if no direct benefit is provided. Required vs. Optional: The guidance differentiates between required biopsies (necessary for trial participation) and optional biopsies (for secondary endpoints or future research), emphasizing that the latter should not create undue burden on participants.
