There isn't a simple answer. In part it depends on why you're struggling. Has something changed unexpectedly? Was the project always unrealistic? What is the stakeholder landscape like? Assuming unexpected change with a reasonable initial plan and reasonable stakeholders it's about talking to them early, replanning pragmatically and communicating transparently.
