Preparation is the key factor, for an upgrade or a change. The assessment is a key piece and thinking about innovative solutions there are interesting AI efforts in this areas, from reverse engineering processes based on the code from code to almost seamless code migration solutions, reducing the risk and effort in some cases. Having the right people is also important, a mixed team that can deal with the old and the new and can connect the dots because sometimes upgrading involve doing things differently as well. Another important fact, be ready if things don't go the way you expect, with a good BCP and DRP, and don't forget about change management and communication, your tech will make no sense if nobody uses it.

In addition to the risk assessments mentioned in other posts, conduct a lessons learnt activity before you start. Analysis of other completed IT projects can provide guidance on what works, what not to do, etc. Traditionally, this activity is usually done at the end of the project, but it should be done at the start as well. It is amazing how many times documented issues end up repeating themselves in multiple projects.

I think the best way to alleviate risks in performing system upgrades is to use a phasing approach to rollout out the changes. If it is a large change, you should use an Agile approach to segment the work into smaller parts and ensure that there is robust testing during your sprints. And most importantly as with any change, have a strategic back-out plan in place if deemed too risky or unsuccessful.

Creo que lo principal es tener una arquitectura donde el alto rendimiento y la alta disponibilidad de las aplicaciones esté asegurada. Evitar tener afinidades maquina-aplicación. Eso para empezar. Tener varios ambientes( environments) donde practicar cualquier progreso hasta llegar a producción. Muchas tecnologías están apostando por un continuous delivery para hacer menos disruptivos los cambios, e incluso evitarlos. Por supuesto tener políticas de disaster recovery process, mediante réplicas sincronas y asíncronas. En resumen, apoyarse en una arquitectura TI que garantice la continuidad del negocio y la producción.
