Having had to deal with this same issue in just about every company and CEO I have coached or consulted, the basic answer is, does the company have SOP's - Standard Operating Procedures with regular review dates with each individual in each department? If each person in the communication chain knows the SOP. How and when to use them, and is held accountable to apply them as outlined. 95%+ of communication issues are solved. I know, you think it is over simplified... until you actually establish SOP's and execute them.

It is often seen that some people has brilliant ideas, that needs to understand and discussed. Sometime those brilliant ideas do not match with the defined goals. Here it is important to make it understand to every one in the team, what is ultimately needed. Some time it is left to the team, to do it there own way. This is not bad as long as team understands the goals, properly and fully.
