Let's keep it simple. Literally. Step one: Connect. Stop the hubbub at the computer, put the iPad down. Connect. As an advocate for vulnerable populations, I believe our goal with everyone is to hold space. It is in coming into the moment, and taking care to understand the complexity behind complex medical needs that we can begin to triage, info share, soft-transfer, and refer. But first, let's just sit down and listen. That's the simple secret.

I agree with the assessments to determine level of care and resources needed. This needs to be coupled with the medical insurance a client has, private resources, as well as determining who is the client’s support system. Interview the patient and review records to see the most acute need. Then research area specialists for the need and make an appointment.Another thing to research would be any programs to help and/or assist the patient with any of their needs. Keep in contact with patient and family to address any needs as they may arise. Most patients appreciate the communication the most because they don’t want to feel alone in the illness.

I make sure that I never give any information to clients re. medical concerns unless they are seeing a provider for specialty care. My scope is narrow, I can talk about connections between mental and physical health, and the possible impact of medication. I make sure that whether it's follow up with a psychiatric care provider, or if they have health issues that mimic MH concerns that they rely on a physician or appropriately trained individual for care as indicated.
