How do you encourage learning from mistakes in your agile team?

Errar es humano, pero aprender de los errores es no reincidir en los mismos. Desde mi experiencia, la clave reside en establecer una cultura donde el error sea visto como una oportunidad para aprender. Esto solo es posible mediante un análisis de falla que evite soluciones superficiales. Personalmente, valoro el método de las '5 M' (Maquinaria, Método, Mano de obra, Medio ambiente, Materia prima), ya que nos direcciona a investigar todas las causas y lo complemento con los '5 porqués'. Por ejemplo, si el problema fue de método, es importante preguntar: ¿El procedimiento estaba documentado? Y si lo estaba, ¿era claro? ¿Estaba actualizado? ¿Era accesible? ¿Se socializó con el personal? Y si fue así, ¿cuándo fue la última vez que se socializó?

We need to support growth mindset in a companies though building safe environment in order to support continuous testing and learning, i.e. Empiricism. Learning through experience, or empiricism, is one of the main tenets of the agile way of thinking. A culture of experimentation must be fostered — a return to our childhood, when a sense of curiosity, freedom and fun were part of most games we played as kids. But none of that will happen without a safe environment; an environment where fear has no place.

In my experience 1. One should not allow the feeling of guilt in the team or individual when the mistakes happen. Mistakes can happen from anyone in the team 2. The members involved should avoid the blame game and members getting the felling of victimized. 3. Embracing challenges fosters learning and growth, while learning from criticism fuels improvement. With determination and persistence, one can become proficient in their chosen field of endeavor. These principles, integral for effective agile teams, emphasize dedication, resilience, and openness to feedback.

During retrospectives, I will discuss what went wrong without blaming anyone, and collaboratively explore how to avoid similar issues in the future. It has become very fashionable to state that “Agile is dead” or that “Scrum doesn’t work”, and I have seen recently some people claiming that “Scrum is not Agile”, which is completely non-sensical! Scrum is one of several so-called “lightweight” approaches to software development of the late 90s, whose founders wrote the Agile Manifesto. Using Scrum as an example of a true Agile approach, we can clearly and easily identify learning loops or feedback cycles in the form of daily scrums, sprint reviews and retrospectives. When this simple framework was designed, and later evolved, its creators knew how essential learning loops were for fast and targeted feedback.

Let the group try new things, assist them in overcoming obstacles, and open up opportunities for learning. This will motivate kids to grow in a secure setting by learning from their errors. Foundational to a "Whole Team" is trust. Having confidence in the team to perform well is one part of this. Allowing team members to make errors and grow from them is another facet of trust. If the team members are: - Not making mistakes OR are not allowed to - Not enabled to learn from mistakes OR are criticized eventually the team would make fewer mistakes at the expense of limited learning & lesser value delivery. In the world of Agile Delivery: - A "Whole Team" is built on trust. An aspect of this is to have faith on the the team to do right. Another aspect of trust is letting team members make mistakes and learn. - An energized team has enough time to do the job, & freedom to make mistakes & learn from them. Allow the team to experiment, help them overcome impediments, create learning avenues. This will encourage them to learn from mistakes and improve in a safe environment.

Existen varias estrategias que funcionan: - Genere seguridad sicológica, que todos se sientan libres de compartir sus errores y aprendizajes - Pregunte a todos que podemos aprender de eso que nos pasó y cómo podemos evitar que vuelva a suceder - Evite el juego de la culpa, asuma usted toda la responsabilidad del error y colaborativamente con el equipo genere la solución y el sistema para que no vuelva a suceder. - Realice análisis de causa raíz, y que esto haga parte de las acciones de mejora de sus retrospectivas.

I have witnessed with concern a growing trend valuing efficiency over effectiveness among some Agile practitioners. The victims are often decreasing levels of innovation and learning for the sake of fast flow of features down the production line — when we least expect we might be back to the Taylorist days of the monumental development methodologies of the last century. The reasons and factors that motivated the early Agile pioneers are even more prevalent today. When we talk about learning in Agile teams I feel now is the time to go back to basics, it is time to remember what Agile really stands for: better products, constant learning and improvement, and always, happy customers.

It can be incredibly motivating to acknowledge and celebrate moments when the team has learned from their mistakes and used those lessons to achieve success.
