As per my opinion and experience: -To identify a hazard in any workplace employees need a basic training to recognize the hazards. -Provide employees convenience to report such as BBSP (Behavioural Based Safety Program) and authority to Intervene and Stop, if they see something is going unsafe. - As safety is everyone's responsibility, personal attitude is a key factor in an organization to report and comply with the policies that the company set for their employees. -Safety culture and Management interest towards the HSE can play a huge role in the hazard and risk measurements such as; an employer should enforce their operatives to identify and report the unsafe acts and conditions in the way of Near Miss Reports, Stop Card Reports etc....

Employees need to know what hazards are, the red flags and the reporting mechanism too. Reporting mechanism which includes incident report forms must be detailed to capture all relevant information but must be kept simple for better understanding of all.

For this, I would try to refrain from using terms such as: "witness statement" "investigation" or any term that makes it seem like the meeting is about finding blame rather than cause and solution. So instead of witness or employee statement, I would call it the operator's perspective. Instead of investigation, I would use root cause analysis or other like terms.
