Any issue can arise at any stage. Here are some suggestions. 1. Review the data sheets of the major components again. Sometimes, an overlooked guide may be the culprit (e.g. attaching any IO to boot-strapping pins of the controller) 2. By designing PCB layout in way to add maximum test points, So they could be checked at PCB manufacturing process. 3. Manufactured PCB is not guaranteed to be perfect. Many times if the tracks are at limit proximity with each other, they might exhibit strange behavior. So never test the limit of the manufacturer.
