First, assume that accidents happen and do not treat the team member like they deserve to be punished, trust me they are already annoyed enough with themselves that you may need to do things to help boost their confidence again. It will also stop other staff being scared to come forward if they make a mistake in future! If staff start to hide things to cover their behinds, then worst leaks could happen without you knowing! Talk to your staff, put safeguards in place, look at processes and how it happened. Reduce the risk of it happening again and make sure staff know that they can come forward safely if it does!
