From the starting point of every project, I communicate clearly with the client about the number of changes agreed within initial budget, and what are the costs of extra changes. Here, for a last minute request I would take some time to re-explain what are the implications in terms of time & price (not only conception fees, also for final execution). If that is clearly understood by the client, then, expectations are set and some design reviews can be applied.

Sometimes the client just needs to see a starting point so they can visualize this type of communication on their project. During the course of discussing the major revision, we iron out all of their concerns, clarify areas that need to be addressed and reassure the client we have a working solution by working together. Have the tough conversation that after this major revision, any and all other major revisions deviate from the scope and will be charged extra hourly rates as necessary. Sometimes that just happens but communication needs to be ever present.

If the first option is not accepted, we will propose a draft proposal (which is not the final standard version) for the client's review. This will help the client understand the baseline expectations for the proposal.
