Define what is "effective"... is it 68%? is it 98% or is it 100%? If you define "effectively addressing" as 100%, then that entails identification of all the problems in the problem set (P1, P2, ... PN)... requiring your team to identify all elements in the set of PROBLEMS and all elements in the set of EVENTS. (The challenge here is identifying the linkage between the elements in the set of P to those in the set of E. Most operations don't care about comms breaking down until there is a problem. When there is a problem (say P1), the AAR will identify the root causes around P1 and not the entire set of problems downstream of event (say E1) that provoked P1 in the first place. What does that mean? It implies "suboptimal" addressing.
