In my case, I know how long it takes to usually complete specific deliverables and I use those estimated times and block them on my calendar. And as for keeping clients happy, of course that also means being able to deliver quality service. For this, I'd make sure to add breaks in between my tasks to give my mind and body enough time to recuperate and avoid accidentally "mixing up" client work.
