This sound like a bigger problem than just a human error. I will tackle this issue by looking at different areas that can possibly contribute to such negligence. 1. Managerial Intervention: We should have clear company policies that backs the adherence to work procedures. This needs to be reviewed and presented to all employees and a refresher workshops in a yearly intervals minimum. This will create awareness and push accountability amongst employees. 2. Cultural Interventions: we need to foster the culture of testing and quality checks that it becomes second nature to all team members. 3. Technical Inventions: Create a feedback loops that will flag missed test, checks or incorrect test inputs.

In my opinion, I believe that people must be train to make right, and we have confidence in them, once that were trained, it's not necessary to worry , but in other case I see that's necessy to measure the process, if and when possible!!!!
