An upgrade failer means a mistake during preparation or configuration or missed a system test before enabling it. 1. Review the current situation and determine whether it is possible to roll back the system before the update; we recommend testing it first and planning it after working hours. 2. Testing is an excellent option to help understand what causes the problem. 3. If it is a severe issue, set up a team of personnel and create a plan to approach the problem; test and copy the system to analyse and test possible starting points and solutions. 4. If an unsuitable update works, live with it for a time until a solution is found, but please reach out to an external organisation to review possible ways to push the upgrade.

One thing I find useful is to take time for proper preparation, everything will run smoothly if you clearly understand the whole picture, system wide, backing up configurations in advance, making sure you have the backup strategy or snapshots in place, all necessary software involved and in the proper locations, communication methodology and of course the rollback plan. Documentation and preparation are keys for success and communication will complement for sure. 90% of our success is because of the actions that we take before the actual implementation.
