In my experience, the best way to manage conflicting preferences and opinions is to really listen and determine where those preferences are coming from. At that point, I try to incorporate a change set that shows a sensitivity to those preferences. I find that most clients really just want to see that they have been listened to and have had input into the final product. That isn't to say that sometimes, your own better judgement as a professional can't prevail, but when that is the case you have to be clear and transparent about your reasoning. Good clients who respect you will understand that. This approach has traditionally won me new advocates and clients.
